Key Responsibilities
- Provide on-site endocrinology consults in nursing home settings, including: o Initial assessments for new admissions and complex endocrine conditions (e. g., diabetes, thyroid disorders, osteoporosis, adrenal disorders) o Routine weekly follow-up visits and care adjustments based on findings.
- Conduct comprehensive evaluations: o Gather medical histories and perform endocrine-focused physical exams o Order and interpret labs including A1C, thyroid panels, metabolic panels, bone density tests, and other diagnostics.
- Collaborate with endocrinologists, interdisciplinary teams, nursing staff, and facility providers to develop treatment plans, manage complex metabolic conditions, and reduce hospital readmissions.
- Administer and manage medications including insulin and endocrine therapies, monitor treatment responses, and adjust regimens appropriately.
Requirements
- Minimum of 6 months of experience in a long-term care or skilled nursing facility (Hospital/outpatient office experience will also be considered).
- Active and unrestricted NP license
- DEA license required
- CPR certification (BCLS/BLS)
- Master’s degree in Nursing (NP applicants).
- Graduation from an accredited NP Program.
- Preferred: Experience in endocrinology (training available if not).
