Hospice Registered Nurse Case Manager (RNCM)

Key Responsibilities

The RN Case Manager coordinates and provides hospice nursing care for an assigned caseload, managing symptoms and supporting patient goals of care. They work closely with the interdisciplinary team, physicians, and families to ensure high-quality end-of-life support.

Requirements

Candidates must hold a current Registered Nurse license in Pennsylvania and possess at least one year of hospice nursing experience. Strong clinical judgment, organizational skills, and the ability to travel throughout the assigned service area are required.
