Job Description

Licensed Hospice Social Worker (LBSW, LMSW, or LCSW)

Victoria, TX | Full-Time | Salary: $78K - $80K DOE.

What You'll Do

  • As a valued member of our interdisciplinary hospice team, you'll:
  • Provide emotional support and counseling to patients and families navigating end-of-life care.
  • Complete psychosocial assessments and develop individualized plans to meet each patient's unique needs.

Connect patients and families with community resources, advance care planning, financial resources, and supportive services.

Serve as an advocate, educator, and trusted guide throughout the hospice journey.

Collaborate with nurses, physicians, chaplains, aides, and other team members to provide exceptional, holistic care.

What You'll Bring

  • Required Qualifications:
  • Master of Social Work (MSW) from an accredited program.
  • Current Texas Social Work license (MSW).
  • Minimum of one (1) year of healthcare experience (hospice or home health preferred)
  • Strong communication, organization, and relationship-building skills
  • Current CPR certification (or willingness to obtain)
  • Valid driver's license, reliable transportation, and automobile insurance
