Key Responsibilities

The provider will perform full-scope family practice services including diagnosis, treatment, and patient care coordination. They are also responsible for maintaining electronic health records and participating in health center quality and safety programs.

Requirements

Candidates must hold a D.O. or M.D. degree, have completed a primary care residency, and possess board certification. A minimum of two years of outpatient primary care experience and current Illinois licensure are preferred.
