Schedule: Mon-Friday 8-4:30 pm. Every fourth weekend.

Our Case Management team supports patients across a variety of acute care
settings, partnering closely with nursing, providers, social work, and other
multidisciplinary team members to help ensure safe, effective care coordination.

How you’ll contribute

A Case Manager RN who excels in this role:

* Monitors and manages patient care to promote continuity, optimal outcomes,
satisfaction, cost efficiency, and regulatory compliance.
* Consults regularly with nursing staff and the multidisciplinary team to
evaluate patient status and appropriateness of care, including admission,
length of stay, transfers, and discharge.
* Leads and coordinates discharge planning, including transfers to other
facilities, referrals to community resources, and discharge education.
* Monitors patient and family satisfaction; responds to questions and concerns
from patients, families, and payors.
* Performs chart reviews to identify and address issues related to service
delivery, outcomes, compliance, cost, and reimbursement.
* Communicates effectively with providers, payors, and ancillary services to
remove barriers and ensure timely care progression.
* Accurately documents case management activities, plans, and outcomes.
* Participates in performance improvement and required continuing education.
* Supports department operations by working alternate shifts/units as needed;
may be assigned on‑call.
* Performs other duties as assigned.

What we’re looking for

* Applicants should have a current RN license to practice in NC (NC licensure
or multi-state (compact) license). 
* Associate’s Degree in Nursing required, Bachelor’s Degree in Nursing
preferred.
* 2+ years of Case Management experience in an Inpatient/Acute Care setting
required.

Additional requirements include

* Strong critical thinking and decisive judgment
* Decisive judgment and the ability to work with minimal supervision. 
* Must be able to work in a stressful environment and take appropriate action.
* Proficiency with multiple clinical software systems

Key Responsibilities

The Case Manager RN monitors and manages patient care to ensure continuity, optimal outcomes, and regulatory compliance. They lead discharge planning and coordinate with multidisciplinary teams to facilitate safe and effective care transitions.

Requirements

Applicants must hold a current Registered Nurse license in North Carolina and possess at least two years of case management experience in an acute care setting. An Associate's Degree in Nursing is required, while a Bachelor's Degree is preferred.
