Empath Home Health, a member of Empath Health is seeking a compassionate PRN Home Health Physical Therapist (PT) to work with us to provide exceptional and individualized care to patients in Fort Myers, FL & surrounding areas.

Locations: Home health visits in the Fort Myers area / Lee county (including Fort Myers, Cape Coral, Lehigh Acres, Estero, Bonita Springs, and surrounding areas). It is flexible - you choose when & where you want to work!

Schedule: PRN; 5-10+ home health visits available per week

What You'll Do

• Perform comprehensive patient assessments, including OASIS assessments at required intervals
• Develop, initiate, and revise individualized plans of care as patient needs evolve
• Deliver skilled occupational therapy services and treatments as outlined in the plan of care
• Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies
• Collaborate with the interdisciplinary team to ensure seamless, holistic care

What You'll Need

• Active Florida PT license
• Minimum of 1 year experience as a licensed Physical Therapist (PT)
• Home health care experience strongly preferred.
• Strong assessment, critical thinking, and patient education skills
• Willingness to participate in weekend and holiday on-call rotations
