About the Role

MedPOINT Management in Sherman Oaks, CA is looking for a dedicated Re-Admissions Case Manager to join our growing healthcare team. In this impactful role, you'll play a critical part in coordinating patient care transitions and reducing preventable hospital readmissions. Responsibilities

• Monitor and manage high-risk patients post-discharge to prevent unnecessary hospital readmissions
• Coordinate care transitions between hospitals, skilled nursing facilities, and outpatient providers

Conduct telephonic and in-person patient assessments to identify barriers to care and social determinants of health

Collaborate with interdisciplinary care teams including physicians, social workers, and utilization management staff

• Develop and implement individualized care plans focused on reducing readmission risk
• Track and document case management activities in electronic health record (EHR) systems
• Educate patients and caregivers on discharge instructions, medication adherence, and follow-up care

Requirements

• Active RN or Licensed Clinical Social Worker (LCSW) license in the state of California
• 2+ years of case management, utilization management, or discharge planning experience
• Strong knowledge of managed care, IPA, or health plan operations preferred
• Experience with care transitions, readmission reduction programs, or chronic disease management
• Proficiency with EHR/case management software and Microsoft Office Suite
• Excellent communication, critical thinking, and organizational skills
• CCM (Certified Case Manager) certification is a plus
