Key Responsibilities
The Registered Nurse Case Manager develops comprehensive care plans to facilitate safe transitions between the hospital and home. They also coordinate clinical communication among the multidisciplinary team and ensure documentation integrity for patient medical records.
Requirements
Candidates must hold a current Registered Nurse license from the Wisconsin Board of Nursing or a compact state. A diploma from an accredited nursing school is required, while BLS certification and CCMC credentials are preferred.
