Key Responsibilities
The Nurse Practitioner will provide primary and specialty care to terminally ill patients in their homes, including managing treatment plans and performing diagnostic tests. They will also collaborate with the interdisciplinary team to coordinate care and provide education to patients and their families.
Requirements
Candidates must be a graduate of an accredited nursing program with national board certification as a Nurse Practitioner and hold a valid state license. A minimum of two years of experience as a registered nurse practitioner is preferred, along with reliable transportation and a valid driver's license.
