Key Responsibilities
The Registered Nurse Case Manager provides holistic, individualized care to patients in their homes and collaborates with a care team to develop personalized care plans. They are responsible for symptom management, administering treatments, and leading end-of-life conversations with patients and families.
Requirements
Candidates must hold an Associate, Bachelor, or Master degree from an accredited nursing program and possess a valid RN license. At least one year of recent acute care experience is required, along with a valid driver's license and reliable transportation.
