Nurse Practitioner (Full-Time)

Local travel is an essential part of this role, with approximately 75% travel required to support patient care needs and collaborate with healthcare partners throughout the community.
What You'll Do
Perform comprehensive patient assessments, physical examinations, and medical history reviews.
Order, interpret, and evaluate diagnostic testing, including laboratory studies and imaging.
Diagnose and treat acute and chronic vascular conditions.
Develop, implement, and manage individualized treatment plans.
Prescribe medications and monitor treatment effectiveness.
Educate patients and caregivers on health conditions, preventive care, and treatment plans.
Collaborate with referring providers and interdisciplinary care teams to ensure seamless patient care.
Maintain accurate and timely clinical documentation.
Monitor patient progress and adjust care plans as needed to achieve optimal health outcomes.
What We're Looking For
Experience practicing as an Advanced Practice Nurse in clinical settings such as primary care, hospitals, outpatient clinics, emergency medicine, or related healthcare environments. New APN graduates considered.
Strong clinical assessment and diagnostic skills.
Knowledge of healthcare regulations, quality standards, and patient care best practices.
Exceptional communication and patient education abilities.
Ability to work independently while thriving in a collaborative team environment.
Excellent critical thinking, problem-solving, and decision-making skills.
Commitment to ongoing professional development and lifelong learning.
Bilingual English/Spanish skills are highly preferred.
Required Qualifications
Current and unrestricted Nurse Practitioner license.
Active DEA and CDS licenses.
Current BLS and ACLS certifications.
Valid driver's license and reliable transportation.
Willingness and ability to travel regularly throughout the service area.
