Key Responsibilities
- Serves as a member and of the hospice and palliative medicine interdisciplinary teams.
- Provides critical thinking and clinical judgment to address the goals of care of the patients and families receiving services.
- Provides clinical direction and expertise to the interdisciplinary team.
- Participates in goals of care discussions and development of plans of care that are clinically appropriate and that meet accepted palliative care and Hospice & Community Care standards and guidelines.
Requirements
- Master's Degree in Nursing required.
- Certified Nurse Practitioner license in the Commonwealth of Pennsylvania required.
- 2 years of NP experience required.
- Hospice and/or Palliative Medicine experience preferred.
