Key Responsibilities

• Serves as a member and of the hospice and palliative medicine interdisciplinary teams.

• Provides critical thinking and clinical judgment to address the goals of care of the patients and families receiving services.

• Provides clinical direction and expertise to the interdisciplinary team.

• Participates in goals of care discussions and development of plans of care that are clinically appropriate and that meet accepted palliative care and Hospice & Community Care standards and guidelines.

Requirements

• Master's Degree in Nursing required.

• Certified Nurse Practitioner license in the Commonwealth of Pennsylvania required.

• 2 years of NP experience required.

• Hospice and/or Palliative Medicine experience preferred.
