Job DetailsJob Location: Columbus, OH 43215We are a Federally Qualified Health Center (FQHC) committed to providing high-quality, compassionate care to underserved communities in rural Ohio. We are seeking a Family Nurse Practitioner to join our integrated care team to provide comprehensive primary care services, including health assessments, diagnosis and treatment of acute and chronic illnesses, preventive care, health education, and care coordination.
Responsibilities
Provide comprehensive primary care services to patients of all ages. Perform physical examinations, diagnose and treat acute and chronic medical conditions, and develop appropriate treatment plans. Prescribe and manage medications for Medication-Assisted Treatment (MAT), as appropriate and in accordance with applicable federal and state requirements. Order, interpret, and communicate laboratory and diagnostic test results. Deliver preventive care, including routine screenings, immunizations, and health education. Coordinate care with specialists, behavioral health providers, and community resources. Participate in quality improvement initiatives and support organizational performance measures. Promote a welcoming, respectful, and culturally responsive environment for all patients.

Requirements
Requires current licensure as a Family Nurse Practitioner in the State of Ohio and current DEA registration. Experience in family practice, community health, or an FQHC setting is preferred.
