Key Responsibilities
The Social Worker provides patient assessment, counseling, and discharge planning while acting as a liaison between patients, families, and community agencies. They work within an inter-disciplinary team to apply psychosocial interventions and support patients in their recovery and independence.
Requirements
A Bachelor's degree in Social Work and a current, valid license to practice as a Social Worker in the state of employment are required. Two years of healthcare experience and a Master of Social Work are preferred.
