Join UnitedHealth Group as a Nurse Practitioner Care at Home – No Nights/Weekends, $10K Sign-On in Greenville, South Carolina, United States. $10,000 Sign On Bonus For External Candidates In Home Care, No Nights, Weekends or Holidays This position requires daily travel to patient homes within assigned area. Key Responsibilities
Conduct comprehensive in-home assessments and manage patients through acute medical situations or chronic condition exacerbations. Collaborate with an interdisciplinary team to provide advanced care planning and patient education.
Requirements
Requires active ANCC or AANP certification for NPs or NCCPA certification for PAs, along with an unrestricted state license and driver's license. Candidates should have clinical experience in geriatric or home health settings and proficiency with electronic medical records.
