The Nurse Practitioner will conduct comprehensive in-home wellness risk adjustment assessments for Medicare and other at-risk members, focusing on overall health evaluation to ensure accurate risk adjustment coding for value-based care. Responsibilities also include performing various clinical evaluations, patient education, and coordinating care findings with primary care providers.
Candidates must possess a valid, unencumbered Nurse Practitioner License for South Carolina and have access to reliable transportation for daily travel within a 55-mile radius of Florence County. Previous in-home risk assessment or 1099 (PRN) experience is preferred, along with a minimum of three years of patient care experience.
