Case Manager, RN, Full-Time - East Campus

Key Responsibilities

The Case Manager assists patients, families, and physicians in managing health care plans and coordinates the continuum of care. They also participate in data gathering for regulatory core measures and ensure the efficient use of hospital resources.

Requirements

Candidates must be a graduate of an accredited nursing school and hold a current Tennessee nursing license. At least one year of broad clinical experience is required, with preference given to those with case management or utilization management experience.
