Key Responsibilities

The Case Manager assists patients, families, and physicians in managing healthcare plans and coordinates the continuum of care. They also participate in data gathering for quality measures and ensure the efficient use of hospital resources.

Requirements

Candidates must be a graduate of an accredited nursing school and hold a current Tennessee nursing license. A minimum of one year of broad clinical experience is required, with case management certification preferred.
