- Location: Port Charlotte, FL (Onsite)
- Type: Part Time
- Pay Rate: Starting $40/hour and up (Based on Experience) and Sign On Bonus up to $1000

What You’ll Do
* Conduct nutrition assessments, consultations, and follow-up evaluations.
* Provide individualized nutrition care plans and 1:1 resident consultations.
* Work with diverse clinical conditions including dialysis, diabetes, cardiac care, wound healing, and tube feeding.
* Develop and implement individualized nutrition care plans.
* Monitor nutritional status and recommend appropriate interventions.
* Collaborate with healthcare professionals to improve nutrition outcomes.
* Utilize the Nutrition Care Process to deliver high-quality nutrition care.
* Make a meaningful impact by using Food as Medicine to improve residents' quality of life.
