Key Responsibilities
The Hospice Nurse Practitioner collaborates with the Interdisciplinary Group (IDG) to ensure appropriate evaluation and palliative treatment for terminal diseases, consulting with and educating patients/families on disease processes and end-of-life care. Responsibilities also include performing face-to-face visits at residences and communicating clinical assessment results to the hospice physician.
Requirements
Candidates must possess a current State RN License and an Advanced Practice Nurse License, along with a valid Driver's License and valid Auto Insurance. Preferred experience includes HomeCare HomeBase, and knowledge of hospice criteria is essential for this role.
