Key Responsibilities
The RN Case Manager will assess patient needs and create personalized hospice care plans to ensure comfort and dignity. They will also provide direct nursing care, manage symptoms, and educate families while collaborating with an interdisciplinary team.
Requirements
Candidates must be a graduate of an accredited nursing school with a current nursing license and at least one year of nursing experience. A valid driver's license and insured vehicle are also required for travel within the coverage area.
