In-House Rehabilitation Team | White Blossom Care Center – San Jose, CA

White Blossom Care Center is seeking a dedicated and compassionate Occupational Therapist (OTR) to join our established in-house rehabilitation team. This is an opportunity to work in a high-volume, resource-rich clinical environment where therapists have the space, resources, and support to provide exceptional patient care.

Position Details

  • Position: Occupational Therapist (OTR)
  • Location: White Blossom Care Center – San Jose, CA
  • Pay Rate: $50–$65 per hour, DOE
  • Schedule: Full-Time, Part-Time, or PRN
  • Program: Stable, In-House Therapy Team

Key Responsibilities

Conduct comprehensive occupational therapy evaluations and develop individualized treatment plans based on physician orders and patient needs.

Provide evidence-based occupational therapy interventions focused on improving functional mobility, independence, safety, and quality of life.

Supervise and support Certified Occupational Therapy Assistants (COTAs) in accordance with California regulations and applicable standards of practice.

  • Participate in interdisciplinary care conferences, utilization reviews, and rehabilitation meetings.
  • Complete timely and accurate evaluations, daily documentation, progress notes, and recertifications.

Assess patients for appropriate durable medical equipment (DME) and provide training to promote safety and independence.

  • Educate patients, families, caregivers, and nursing staff on therapeutic strategies and carryover programs.
  • Participate in discharge planning and caregiver training to support successful transitions of care.

Qualifications

  • Current and valid California Occupational Therapist (OT) license in good standing.
  • Experience in post-acute, skilled nursing, or long-term care is preferred.
  • New graduates are welcome and encouraged to apply.

  • Excellent communication, teamwork, and organizational skills.
