About the Role
Join Carespring Health Care Management as a Nurse Practitioner or Physician Assistant – No Call, M-F in United States.
Key Responsibilities
- Interact clinically with Physicians, other NP/PAs, case managers and others involved in the acute care episode to understand patient needs in transitional care.
- Evaluate potential candidates for transitional care management and decide which patients best fit the model of care.
- In dedicated office space, conduct medication reconciliation and educate patients and care givers on medication, plan of care and disease management.
- Work with case managers, PCPs and others to coordinate the transition of care and care services.
Requirements
- Nurse Practitioner License for Kentucky and/or Ohio
Work Schedule
- Full-Time position.
- Call responsibilities required.
- No weekends required.
- Day shift only - no nights.
- Flexible scheduling available.
