Key Responsibilities
The RN Case Manager leads the hospice care team to create and manage patient Plans of Care while providing direct day-to-day nursing care. They are responsible for administering treatments, maintaining clinical records, and coordinating with interdisciplinary teams to ensure patient comfort and quality of life.
Requirements
Candidates must hold a current Registered Nurse (RN) license and possess a valid driver's license with reliable transportation. At least two years of nursing experience is preferred, with recent experience in hospice or home care highly desired.
