RN Case Manager - Staunton Clinic (Part-Time)

Department: Staunton Clinic
Work Hours: 24 hours per week (minimum). This is a flex schedule, days/hours can
change based on coverage needs. No weekends.

Basic Function: Coordination of services, education, advocacy and direct care to
individuals with mental illness/intellectual disabilities and their families who
require supports to support recovery and increase community tenure within the
least restrictive setting across the continuum of care. Provides nursing support
to the psychiatric team completing Triage/screening for all incoming calls
related to medication/symptoms/patient concerns.

Qualifications

Required: Graduate of an accredited school of nursing with current Pennsylvania
license. Successful completion of applicable clearances as outlined in Human
Resources policy HR-106 prior to commencing employment. BLS certification. Valid
Pennsylvania driver’s license. Must have a minimum of 3 years’ experience in a
psychiatric setting. Recent experience in case management, UR, or D/C planning.
Knowledge/ability to assess and utilize community resources.

Preferred: Knowledge of venipuncture, Clozaril monitoring and decanoate
injections.
