The Physical Therapist plans and provides physical therapy services to clients in their homes according to the Plan of Care. They are responsible for performing evaluations, administering treatments, and maintaining accurate clinical records while coordinating with physicians and the care team.
Candidates must be graduates of an accredited Physical Therapy program and hold a current, unrestricted license in Minnesota. At least two years of experience in a healthcare setting is required, along with a valid driver's license and reliable transportation.
