Outpatient Physical Therapist, PT, Senior Living Visits

Key Responsibilities

The Physical Therapist will perform outpatient home health visits to assess patient mobility, strength, and balance while developing individualized care plans. They are also responsible for educating clients and families on rehabilitative care and accurately documenting all interventions using electronic medical records.

Requirements

Candidates must hold a current Pennsylvania Physical Therapist license and have at least one year of recent work experience. A degree from an accredited Physical Therapy program is required, along with strong organizational and computer skills.
