Key Responsibilities
The Physical Therapist Case Manager will perform home-based evaluations, develop comprehensive care plans, and coordinate with physicians to restore patient function. They will also supervise Physical Therapy Assistants and ensure high-quality care delivery in accordance with regulatory standards.
Requirements
Candidates must hold a current, unrestricted California Physical Therapist license and a valid driver's license. Experience in home health care and familiarity with regulatory guidelines are highly preferred.
