Join UnitedHealth Group as a Nurse Practitioner Care at Home – No Nights, Weekends, Holidays in Charleston, South Carolina, United States. $10,000 Sign-on Bonus for External Candidates In Home Care, No Nights, Weekends or Holidays This position requires daily travel to patient homes within assigned area. Key Responsibilities
Conduct comprehensive in-home assessments and manage patients through acute medical situations or chronic condition exacerbations. Collaborate with an interdisciplinary team to provide counseling, education, and advanced care planning for high-risk beneficiaries.
Requirements
Requires an active ANCC or AANP certification for NPs or NCCPA certification for PAs, along with unrestricted state licensure and a valid driver's license. Candidates should have clinical experience in geriatric, home health, or long-term care settings.
