Key Responsibilities

The Nurse Navigator identifies patient financial and medical eligibility while developing and implementing comprehensive care plans to ensure quality, cost-effective patient outcomes. They serve as a liaison between patients, families, and the multidisciplinary healthcare team to facilitate communication, scheduling, and continuity of care.

Requirements

Candidates must be graduates of an accredited nursing school with a current, valid RN license and at least two years of nursing experience. A current BLS for Healthcare Providers certification is also required.
