Key Responsibilities
The Physical Therapist provides skilled therapy services in patient homes, including performing OASIS assessments and developing individualized plans of care. They are responsible for documenting patient progress, educating families, and supervising therapy assistants to ensure high-quality care.
Requirements
Candidates must hold a current Physical Therapist license in the state of employment and possess at least one year of experience in an acute setting. A valid driver's license and reliable transportation are required for travel to patient locations.
