Key Responsibilities
The RN Case Manager will own patient assessments and skilled visit documentation while delivering high-acuity home health care, including wound care and infusion therapy. Requirements
Candidates must hold a current Texas RN license and possess at least 3 years of clinical nursing experience. Proficiency in wound care, infusion therapy, and comfort with field-based work are required, along with a current CPR certification.
