Key Responsibilities
Conduct comprehensive in-home health evaluations, including physical exams and medical history reviews, to bridge gaps in care. Build trusted relationships with patients to improve health outcomes and provide connections to necessary care.
Requirements
Must be an MD or DO with an active, unrestricted license in the coverage area. Ability to travel locally to patient homes and perform diagnostic services is required.
