The Clinical Pharmacist assesses patient suitability for home care, designs and monitors patient-specific pharmacy care plans, and ensures the safe preparation and dispensing of medications. They also provide medication education to patients and staff while maintaining compliance with legal, regulatory, and safety standards.
Candidates must be graduates of an accredited School of Pharmacy with a current, active Pharmacy license. Requirements include at least one year of experience in sterile IV admixture procedures or equivalent clinical pharmacy experience.
