Key Responsibilities
The Nurse Navigator serves as a primary point of contact to guide patients through the healthcare system, coordinating appointments, referrals, and care transitions. They also identify barriers to care and provide ongoing support to ensure patients remain engaged in their treatment plans.
Requirements
Candidates must hold a Bachelor of Science in Nursing (BSN) or an Associate Degree in Nursing (ADN) with relevant experience, along with a current Missouri RN license. A minimum of three years of healthcare experience and current BCLS certification are required.
