Key Responsibilities
The coordinator provides High Fidelity Wraparound and Intensive Care Coordination services to youth and families with complex behavioral health needs. They facilitate team meetings, develop safety plans, and monitor service effectiveness while providing ongoing advocacy and support.
Requirements
Candidates must hold a Bachelor's degree in a human services field, or an Associate's degree with three years of mental health experience, or a High School diploma with five years of behavioral health experience. The role requires the ability to travel regularly within designated Illinois counties.
