Key Responsibilities
The Registered Nurse Case Manager identifies patient needs, develops care plans, and integrates resources to ensure optimal clinical and financial outcomes. They also facilitate care coordination, perform utilization reviews, and manage the discharge planning process in collaboration with the healthcare team.
Requirements
Candidates must hold a valid Massachusetts registered nurse license and possess at least three years of clinical experience in discharge planning or case management. Proficiency in specialized computer-based systems for clinical data reporting and analysis is also required.
