Key Responsibilities
Conduct patient evaluations and manage acute and chronic conditions through a telemedicine platform in an in-person clinical setting. Collaborate with physicians and clinical staff to develop individualized treatment plans and maintain accurate electronic medical records.
Requirements
Must be an active Nurse Practitioner in New York with current Medicare and Medicaid credentialing. Requires strong clinical assessment skills and the ability to manage approximately 25 patients per day.
