Job Description

Summit Healthcare is hiring on behalf of a client. A growing healthcare services organization is seeking an experienced Medical Director to provide clinical leadership and operational oversight for a large network of skilled nursing facilities throughout the New York metropolitan area.

The organization is not a nursing home operator. Instead, it partners with skilled nursing facilities and senior care communities by delivering specialized physiatry, pain management, and related clinical services directly on-site.

Its physicians and advanced practice providers work alongside facility teams to deliver high-quality, patient-centered care focused on pain management, rehabilitation, mobility, functional improvement, and overall quality of life.

The organization maintains an established presence across Queens, Brooklyn, and select areas of Long Island and continues to expand its clinical partnerships.

The Medical Director will serve as the senior clinical leader overseeing medical services across approximately 30–40 skilled nursing facility locations. This individual will direct, mentor, and supervise a team of approximately 10 on-site providers, ensuring consistent clinical quality, regulatory and payer compliance, and operational excellence throughout the network.

This leadership role combines strategic oversight with hands-on provider management and facility onboarding. While leadership and administrative responsibilities are primarily conducted remotely, on-site presence is required across Queens, Brooklyn, and Long Island for provider onboarding, new facility service launches, relationship management, and occasional clinical coverage when necessary.

Key Responsibilities

Clinical Leadership & Quality Assurance

Provide overall medical leadership and clinical direction for physiatry and pain management services across 30–40 skilled nursing facility locations.

Oversee clinical standards, clinical protocols, provider performance, and quality metrics across the facility network.

Ensure documentation quality, adherence to clinical guidelines, and alignment with regulatory standards and payer compliance requirements.

Review clinical outcomes, documentation trends, and quality indicators to identify improvement areas and implement corrective development plans as needed.

Serve as a senior clinical resource for providers managing medically complex residents, chronic pain, rehabilitation plans, functional limitations, and complex post-acute care needs.

Participate in quality improvement initiatives and support the development and expansion of new clinical programs and service lines.

Provider Recruitment, Onboarding & Team Development

Oversee and manage a distributed team of approximately 10 on-site physicians and advanced practice providers (APPs).

Assist with sourcing, interviewing, evaluating, credentialing, and hiring new clinical providers to support organizational growth.

Lead the onboarding, training, and ongoing professional development of new and existing clinical staff.

On-Site Provider Onboarding: Be physically on-site to onboard new providers at newly established facilities, facilitating introductions to facility leadership, nursing staff, clinical workflows, expectations, and protocols.

Provide continuous guidance, coaching, and mentorship to clinical staff, proactively addressing performance, operational, or documentation concerns.

Facility Implementation & Operations

Assist with the clinical implementation, workflow design, and launch of services at newly partnered skilled nursing facilities.

Establish appropriate provider coverage schedules, clinical workflows, protocols, and service expectations when initiating new locations.

Collaborate closely with executive leadership, operations, credentialing, and billing teams to resolve clinical matters and ensure care is delivered efficiently and compliantly.

Serve as the primary clinical point of contact and trusted advisor for facility administrators, Directors of Nursing (DONs), rehabilitation teams, and attending physicians.

Support facility partnership retention and satisfaction by holding providers accountable to high standards of responsiveness, professionalism, and clinical service.

Clinical Coverage & Travel

Direct Clinical Coverage: Provide occasional, direct clinical coverage when necessary, including situations where a scheduled provider is unavailable and alternative coverage cannot be secured.

Travel on-site to facilities throughout Queens, Brooklyn, and select areas of Long Island as needed for facility launches, provider onboarding, clinical oversight, partner meetings, or coverage needs.

Medical Degree: Medical Doctor (MD) or Doctor of Osteopathic Medicine (DO) with accredited postgraduate training in Physical Medicine & Rehabilitation (PM&R), Physiatry, Pain Management, or a closely related specialty.

Licensure: Active, unrestricted medical license in the State of New York is required.

Location & Travel: Candidates may reside in New York, New Jersey, or Connecticut, provided they maintain an active NY medical license and have the ability to travel regularly to facility locations in Queens, Brooklyn, and Long Island as operational and clinical needs arise.

SNF Experience: Must have significant, direct clinical experience working within skilled nursing facilities (SNFs). A deep understanding of post-acute care operations, interdisciplinary workflows, and SNF regulations is strictly required.

Leadership Experience: Prior experience as a Medical Director, Clinical Director, or physician leader managing a team of physicians and/or advanced practice providers is strongly preferred.

Operational & Compliance Acumen: Solid understanding of payer compliance, medical documentation standards, and clinical billing practices in post-acute care settings.

Collaboration Skills: Proven ability to build collaborative relationships with SNF administration, nursing leadership, rehabilitation therapists, and corporate operational teams.

Flexibility: Willingness to provide occasional direct patient care coverage when coverage gaps arise.

Compensation & Position Highlights

Base Salary: $300,000 – $400,000 annually, commensurate with clinical experience, leadership background, and qualifications.

Leadership Scope: Senior Medical Director role with clinical oversight spanning 30–40 facility partnerships and a dedicated provider team.

Impact & Growth: Direct influence over clinical standards, provider development, new facility service launches, and program expansion within a growing healthcare organization.

Work Model: Hybrid structure combining remote administrative leadership with targeted on-site facility presence across Queens, Brooklyn, and Long Island for onboarding, relationship management, and clinical coverage.

Requirements

Candidates must hold an MD or DO degree with postgraduate training in PM&R, Physiatry, or Pain Management and possess an active, unrestricted New York medical license. Significant clinical experience in skilled nursing facilities and prior leadership experience managing clinical teams are strictly required.
