Hospice RN Case Manager ($10k Sign-on Bonus)

Qualifications

Job Qualifications: Education: Graduate of an accredited school of nursing. Current RN license in the state of practice. Hospice and Palliative Care certification is preferred. Experience: Minimum of 2 years of nursing experience, with at least 1 year in hospice or palliative care preferred. Skills: Strong clinical assessment and critical thinking skills. Excellent communication and interpersonal abilities. Proficiency with EMR systems and documentation. Ability to work independently and manage a caseload. Compassionate approach to patient and family care. Environmental and Working Conditions: Works in patient homes and hospice facilities. Requires local travel to patient locations. May be exposed to varying home environments and weather conditions. Physical and Mental Effort: Requires prolonged periods of driving and patient care. Must be able to manage stressful situations with empathy and professionalism. Requires working under some stressful conditions to meet patient needs and regulatory deadlines.
