Key Responsibilities
The Case Manager provides mental health rehabilitation services to clients and families to support recovery and community functioning. Responsibilities include conducting counseling sessions, maintaining clinical documentation, and coordinating care with community resources.
Requirements
Candidates must hold at least a Bachelor's degree in a relevant field such as Psychology, Social Work, or Addiction Studies. Applicants must also qualify as a Mental Health Professional (MHP) or Qualified Mental Health Professional (QMHP) per Illinois state regulations.
