Key Responsibilities
The ambulatory care clinical pharmacist promotes optimal drug use outcomes for complex patient populations within primary care settings. They provide specialized clinical expertise, participate in the pharmacy care team, and contribute to policy development and medication therapy management.
Requirements
Candidates must hold a Doctor of Pharmacy degree from an ACPE accredited college and be a licensed pharmacist in Vermont and New York. At least 3 years of advanced practice experience in ambulatory care or a clinical outpatient setting is preferred.
