The Home Health Occupational Therapist (OT) through Franciscan is responsible for evaluating and implementing a plan of care to improve, restoring and enhancing physical functional performance, and promoting injury prevention. This role, also, performs documentation and quality improvement activities, as outlined in departmental policies and procedures.

• Perform comprehensive occupational therapy evaluations, treatment and related follow-up services including discharge planning.
• Document patient assessments and pertinent information in a timely manner.
• Develop a treatment plan with measurable goals based on patient assessment.
• Provide occupational therapy, including selection and use of adaptive equipment.
• Collaborate with patient and other healthcare personnel or outside agencies regarding coordination of care.

Qualifications

• Bachelor's Degree- Occupational Therapy - Required
• 1 year Occupational Therapy Experience - Required
• 1 year Home Health Experience - Preferred
• Occupational Therapist (OT) State Licensing Board - Required
• Cardiopulmonary Resuscitation (CPR)- American Heart Association- Required
• Driver's License - Required
• Auto Insurance - Required
