Key Responsibilities
The Advanced Practice Clinician conducts comprehensive medical assessments and provides clinical care to high-risk patients in nursing homes and assisted living facilities. They also collaborate with interdisciplinary teams to manage care plans and provide education to patients and their caregivers.
Requirements
Candidates must hold an active ANCC or AANP certification for NPs or NCCPA certification for PAs, along with an unrestricted state license and driver's license. At least one year of clinical experience in a long-term care or geriatric setting is preferred.
