Key Responsibilities
The Nurse Practitioner will provide comprehensive patient care through a combination of home visits and telehealth services for seniors. Responsibilities include managing acute and chronic conditions, developing treatment plans, and providing clinical education to patients and their families.
Requirements
Candidates must hold a Master’s degree in Nursing, an Advanced Practice Nursing license, and NP Board Certification. A valid driver’s license, reliable transportation, and unrestricted DEA prescribing privileges are also required.
