Key Responsibilities
The Nurse Practitioner will perform comprehensive patient assessments and manage acute and chronic conditions through a mix of home visits and telehealth. They are responsible for developing treatment plans, ordering diagnostic tests, and providing clinical education to patients and facility staff.
Requirements
Candidates must hold a Master’s degree in Nursing, an Advanced Practice Nursing license, and NP Board Certification. A valid driver's license, reliable transportation, and DEA licensure for prescribing are also required.
