Description

The Home Health Speech Therapist provides specialized speech and language therapy services to patients in their homes. In this role you will be responsible for evaluating, diagnosing, and treating speech, language, communication, and swallowing disorders. Your role is crucial in developing personalized therapy plans, implementing effective interventions, and working collaboratively with patients and their families to improve communication skills and overall quality of life.

Schedule: PRN, Weekdays, Day Shift

Primary Responsibilities and Duties

Conduct thorough assessments of patients’ speech, language, communication, and swallowing abilities to identify deficits and develop individualized therapy plans.

Design and implement targeted therapy programs to address specific disorders, including speech articulation, language comprehension and expression, cognitive-communication skills, and dysphagia (swallowing difficulties).

Educate patients and their families on therapy techniques, home exercises, and strategies to support and enhance communication and swallowing functions.

Monitor and document patient progress, response to therapy, and any changes in condition, providing detailed reports to the supervising physician and other healthcare team members.

Collaborate with physicians, nurses, occupational therapists, and other healthcare professionals to ensure a coordinated and holistic approach to patient care.

Maintain accurate and up-to-date patient records and documentation in compliance with regulatory and organizational standards.

Participate in team meetings to contribute to comprehensive care planning and decision-making.

Stay current with advancements in speech therapy practices and participate in ongoing professional development.

Perform all other duties assigned.

Requirements

  • Job Qualifications:
  • Valid Speech-Language Pathologist (SLP) license in the state where work is performed.
  • Any experience in a home health or similar setting preferred.

Excellent communication and interpersonal skills, with the ability to interact effectively with patients, families, caregivers, and healthcare professionals.

Strong clinical skills and knowledge of speech and language therapy principles, techniques, and best practices.

Demonstrated ability to prioritize tasks, manage multiple responsibilities, and adapt to changing priorities in a fast-paced environment.

Ability to build rapport with patients through a compassionate and caring demeanor.

Physical Requirements

Ability to lift and transfer patients, with or without assistance, up to 50lbs.

Capability to perform physical tasks such as bending, standing, walking, and climbing stairs for extended periods.

Visual acuity and dexterity for performing tasks such as reading medical records, administering injections, and handling therapeutic tools.

Ability to respond swiftly and efficiently to emergency situations.

Capacity to work in various home environments which may include navigating stairs, tight spaces, or uneven surfaces.

Maintain regular attendance and timeliness as set forth within the work schedule designated by the department supervisor/manager.

Job requires accepting criticism and dealing calmly and effectively with high-stress situations.

The ability to use your abdominal and lower back muscles to support part of the body repeatedly or continuously over time without "giving out" or fatiguing.
