What you’ll be doing
- Make the initial evaluation visit and regularly reevaluate the patient’s nursing needs.
- Meet Daily Productivity Standards.
- Initiate the plan of care and necessary revisions.
- Provide those services requiring substantial specialized nursing skills.
- Initiate appropriate preventative and rehabilitative nursing procedures.
Prepare clinical and progress notes for each patient visit and summaries of care conferences on their patients in a timely manner.

Job Type and Pay
- PRN, Part time and Full-time
- $43-$52/hr
What we’re looking for
- A passion to serve and help others live their best lives possible.
- Graduate of an accredited school of professional nursing.
- Currently licensed as a registered nurse in the state of the job posting.
- Minimum of one (1) year of nursing experience in a home health setting.
- Must have current BLS Certification.
- Access to reliable transportation.
Schedule
- Day shift
- Monday to Friday 8am-5pm
- Weekends as needed
Key Responsibilities
Perform initial and regular patient evaluations to initiate and revise personalized plans of care. Provide specialized nursing services and maintain timely clinical progress notes and care summaries.
Requirements
Must be a licensed Registered Nurse with at least one year of experience in a home health setting. Requires a degree from an accredited nursing school and a current BLS certification.
