Join VNA & Hospice of the Southwest Region as a Physical Therapist in Rutland County, Vermont. VNA & Hospice of the Southwest Region is seeking compassionate and motivated Physical Therapists (PTs) to join our team in Rutland County. We are hiring for full-time and per diem positions working with adult and/or pediatric patients in the home health setting.
Key Responsibilities
- Completing PT and OASIS assessments as appropriate.
- Developing and implementing individualized plans of care.
- Providing therapeutic interventions and caregiver education.
- Monitoring patient progress and adjusting treatment plans as needed.
Requirements
- Degree in Physical Therapy from an accredited program.
- Current Vermont PT license.
- Home health or prior clinical experience preferred.
- Ideal candidates are compassionate, organized, flexible, and comfortable working independently while remaining part of a collaborative care team.
Benefits & Compensation
- Salary: $40.28 - $60.42/hour
- Signing bonus: $10,000.
- PSLF eligible (Public Service Loan Forgiveness).
