Join VNA & Hospice of the Southwest Region as a Physical Therapist in Rutland County, Vermont. VNA & Hospice of the Southwest Region is seeking compassionate and motivated Physical Therapists (PTs) to join our team in Rutland County. We are hiring for full-time and per diem positions working with adult and/or pediatric patients in the home health setting.

Key Responsibilities

  • Completing PT and OASIS assessments as appropriate.
  • Developing and implementing individualized plans of care.
  • Providing therapeutic interventions and caregiver education.
  • Monitoring patient progress and adjusting treatment plans as needed.

Requirements

  • Degree in Physical Therapy from an accredited program.
  • Current Vermont PT license.
  • Home health or prior clinical experience preferred.
  • Ideal candidates are compassionate, organized, flexible, and comfortable working independently while remaining part of a collaborative care team.

Benefits & Compensation

  • Salary: $40.28 - $60.42/hour
  • Signing bonus: $10,000.
  • PSLF eligible (Public Service Loan Forgiveness).

  • Full-Time position.
