The Physical Therapist is responsible for evaluating, developing, and implementing patient treatment plans while providing skilled therapy in a home health setting. They also serve as a case manager, supervising clinical staff and coordinating care with the interdisciplinary team.
Candidates must hold a Bachelor's degree in Physical Therapy from an accredited institution and possess a valid driver's license. At least two years of professional physical therapy experience is preferred.
